4-week starter protocol
This is the plan to print. It is not a promise. If the screen says tight, you will not “tough out” four weeks of kegels.
Quick answer
Week 0: decide tight vs weak and find the muscles. Weeks 1–4: build from 3-second holds to about 10 repetitions, three times a day, with full rest, only if kegels are appropriate. Stop for pain. Week 0 — Screen and find
- Read tight vs weak. Pain, hesitant stream, or kegels-made-it-worse → reverse kegels only.
- Find the lift using these cues. One urine-stop test maximum.
- Red flags → clinician, not this page.
If kegels are appropriate

If reverse kegels are appropriate
2–3 sessions a day of 8–10 belly breaths with a pelvic drop. No bearing down. Reassess with a PT before adding strengthening.
Stop rules (print these)
- Pain during or after
- More urgency, more hesitation, worse erections
- No let-go between reps
- Catheter in, blood in urine, retention, fever, neurologic symptoms

Is this 4-week kegel plan a clinical trial protocol?
No. It is an editorial synthesis of Mayo Clinic, Cleveland Clinic, and MSK dosing, started easier for beginners. It has not been tested as this exact calendar.
What if I need reverse kegels instead?
Skip the strengthening weeks. Use the week-1 screen. If you map to a tight floor, follow reverse kegels only and get a PT. Do not complete a strengthening plan to be consistent.
Can I print this as a PDF?
